You can now order prescriptions and make appointments online through Patient Access or the NHS App. To get online access with the practice you will need to fill in the form below and show some ID to verify your identity. If you do not have valid ID we can use alternative methods and we can also verify ID by video call.
Completed forms can be returned via email to holmsidemedicalgroup@nhs.net or dropped in at the surgery
