Practice Information

How to Register

Your practice has an open list and welcomes requests for registration from patients who live in or are moving into the practice area. To register as a patient please present your medical card or complete form GMS1 available from reception or even easier, register on-line via Patient Access!

Patients are registered with the practice rather than a particular doctor.

Home Visits

Home visits will be made only when there is a genuine medical inability to get to the surgery.

Home visits will not be carried out for social reasons which include access to transport.

Please telephone before 10.30am except in an emergency, to discuss your home visit request. A GP may return your call to assess the severity and urgency of your need.

Same day help for your poorly child

If your child has become unwell in the last few days and you would like urgent advice then please contact the surgery and we will arrange for a clinician to speak to you and if necessary to see your child, today – this could save you an unnecessary trip to A+E or a Walk-in Centre. Can we also make you aware of the Little Orange Book, which provides expert help for parents and carers on how to manage common problems in babies and small children – ask your GP for a copy, or view it online here

Test Results

Please telephone the practice (0191 273 4009) between 2:30pm and 4:30pm, Monday to Friday. Should you call outside of these hours, you will politely be asked to ring back during the appropriate times.  An alternative way to know your test results is to register for on-line access!

The practice will contact patients by telephone or letter regarding any tests which may warrant further investigation or treatment. Patients with ‘normal’ results will not be routinely contacted.

Please note that we do have a strict policy regarding confidentiality and data protection. In this respect we will only give out results to the actual patient they relate to unless the patient has given prior permission.

Patient Transport Services

Please contact the North East Ambulance Service direct on 0191 215 1515.

Text Message Reminders

As well as Mjog text message service for appointment reminders, we are now using Chain SMS to message patients. It means we can be much more proactive about some communications, messaging you quickly and securely. Some examples of how we might use it are: reminders or notifications (e.g. your prescription  is ready) or letting you know we tried to call you. The Practice name will always be at the bottom of the message and you won’t be able to reply to them. Your mobile number has to be update as it only works with a mobile telephone number not a landline.

To take advantage of this service, please ensure we hold an up-to-date mobile telephone number for you. We will send a text reminder 2 days prior to your appointment.

Call Recording

Calls to and from the practice will be recorded for training purposes.

Zero tolerance

The practice operates a zero tolerance policy with regard to violence, abuse or aggression. Any visitor to the surgery who is violent, abusive or threatening towards any member of staff, patient or other visitors will be asked to leave the premises or the police may be called. If the person is a patient of the practice this could result in removal from the practice list. The police will be called in the event of a refusal to leave.

Patient Participation Group

The Practice would like to invite you to join our Patient Participation Group

The group will be made up of 2 components:

  • A core group of volunteers who will meet half yearly with Practice staff. Whilst the core Group is open to members, we are particularly interested in attracting the views of those patients in the 16-45 age group. In order to maintain informality and open discussion, the group will be limited to 10-12 people.
  • A virtual group of patients who are happy to give their e-mail details to the Practice and be consulted electronically on specific issues as they arise. There are no limitations on membership of this group, ideally the bigger the better.

If you are interested in contributing to the development of your surgery and its services, please contact reception for a registration form or Register Here.
