How we use your medical records:
- Holmside Medical Group handles medical records according to the laws on data protection and confidentiality.
- We share medical records with health professionals who are involved in providing you with care and treatment. This is on a need to know and event by event basis.
- Some of your data is automatically copied to the Shared Care Summary Record; a secure government database of patients’ basic medical information. It is used to reduce errors and avoid delays if a patient requires treatment outside of the surgery. It can only be accessed by authorised personnel involved in the patient’s direct care.
- We share some of your data with local out of hours / urgent or emergency care services so that, in an emergency, you can receive the best care possible.
- Data about you is used to manage national screening campaigns such as Flu, Cervical cytology and Diabetes prevention.
- Data about you, usually de-identified, is used to check and manage the quality of care provided by the NHS.
- We share information when the law requires us to do so, for instance; when we are inspected, reporting certain illnesses or safeguarding vulnerable people.
- We may also share medical records for medical research to try and improve treatments and patient care.
- We retain and destroy records in line with the guidance set by the Information Governance Alliance.

Minuteful Kidney – Privacy Policy September 23-24
Purpose:
To inform patients about the service collaboration between the practice and Healthy.io. Patients can be directed to the GP Practices Privacy Notice should they wish to understand more about the service.
Minuteful Kidney service for patients with diabetes (and/or other conditions) The data is being processed for the purpose of delivery of a programme, sponsored by NHS Digital, to monitor urine for indications of chronic kidney disease (CKD) which is recommended to be undertaken annually for patients at risk of chronic kidney disease e.g., patients living with diabetes. The programme enables patients to test their kidney function from home. We will share your contact details with Healthy.io to enable them to contact you and send you a test kit. This will help identify patients at risk of kidney disease and help us agree any early interventions that can be put in place for the benefit of your care. Healthy.io will only use your data for the purposes of delivering their service to you. If you do not wish to receive a home test kit from Healthy.io we will continue to manage your care within the Practice. Healthy.io are required to hold data we send them in line with retention periods outlined in the Records Management code of Practice for Health and Social Care.
