Holmside Medical Group is proposing to extend its practice boundary to include nearby areas. We are currently seeking feedback from patients, local residents and stakeholders before any decision is made. If the proposal is approved, the following areas would be included within the new practice boundary: • Callerton • Black Callerton Maps of the current and proposed boundary are available at reception and below. The shaded area represents our current boundary, the orange lined area shows our projected extension. The proposed boundary extension is intended to ensure that patients living in newly developed housing areas have access to local GP services. Holmside Medical Group has a long-standing history of providing primary care to the surrounding community, and these new developments are a natural extension of the population we already serve. The proposed change simply incorporates these new residential areas, which currently fall just outside our existing practice boundary, allowing us to continue delivering accessible, high-quality care to our local community. Managing Referrals and Appointments in the NHS App

You can use the NHS app to view, confirm and change the details of any upcoming hospital referrals from your GP.

  • Open the NHS App and make sure you are logged in
  • Select Appointments
  • You will see optons for appointments at your GP practice and Hospital
  • You may be able to Book an appointment at your GP practice. You can Manage GP appointments to cancel or reschedule.
  • You will see details of any hospital or special appointments. Select for options to cancel or reschedule.
  • Select Referrals to see any specialist care you have been referred to
  • Depeding on the hopsital or care provider, you may see information on Waiting Lists, or any action you need to take – this is called Patient initiated follow up (PIFU)
