Medical Records
Every patient registered with a GP practice has the legal right to access their medical records for free.
Your records may include appointment history, test results, referral or hospital discharge letters, vaccination or childhood immunisations.
You can also nominate someone you trust to have access to your records if you need to.
Records are available digitally through our online service, Patient Access and the NHS App.
If you already have access to Patient Access, you can access your records with the button below. Our Reception team can set up access if you haven’t registered previously – call them on 0191 273 4009.

The proposed boundary extension is intended to ensure that patients living in newly developed housing areas have access to local GP services. Holmside Medical Group has a long-standing history of providing primary care to the surrounding community, and these new developments are a natural extension of the population we already serve. The proposed change simply incorporates these new residential areas, which currently fall just outside our existing practice boundary, allowing us to continue delivering accessible, high-quality care to our local community.
