The National Diabetes Experience Survey is a new national survey of people living with type 1 and type 2 diabetes which has been commissioned by NHS England.
NHS England plans to use the information gathered from this survey to make informed policy and practice decisions about how best to support patients at both national and local level.
If you would like to opt-out of being selected to take part in this survey, you must inform the NHS before the 17th January 2024. The ways to opt-out are listed in the poster below
